Responsibilities
- Conduct comprehensive assessments of client needs, strengths, and challenges to develop personalized care plans.
- Provide direct counseling, crisis intervention, and emotional support to individuals and families facing housing, financial, or mental health crises.
- Advocate for clients' rights and access to essential community resources, including healthcare, legal aid, and employment services.
- Maintain accurate and confidential case records in accordance with professional standards and regulatory compliance.
- Collaborate with interdisciplinary teams, including healthcare providers, police, and school officials, to ensure holistic client support.
- Facilitate support groups and workshops to empower clients and foster community connections.
- Monitor client progress and adjust treatment strategies to ensure positive outcomes.
Qualifications
- Master of Social Work (MSW) degree from an accredited university, with a specialization in Community Services or Mental Health preferred.
- Valid registration or licensure with the College of Social Workers and Social Service Workers (CSWSSW) in Ontario.
- Minimum of 3-5 years of relevant experience in social services, case management, or community outreach.
- Strong knowledge of relevant legislation, including the Child and Family Services Act and the Accessibility for Ontarians with Disabilities Act (AODA).
- Exceptional interpersonal communication skills with the ability to build rapport with diverse populations.
- Proficiency in using case management software and Microsoft Office Suite.
- Bilingual (English/French) is an asset.
