Responsibilities
- Conduct comprehensive psychosocial assessments to determine client needs and eligibility for services.
- Develop and implement individualized case plans in collaboration with clients and their families.
- Provide crisis intervention and emotional support during difficult life transitions or emergencies.
- Advocate for client rights and facilitate access to necessary community resources, healthcare, and housing.
- Monitor client progress and adjust treatment plans to ensure positive outcomes.
- Maintain accurate and confidential case documentation in compliance with legal and organizational standards.
Qualifications
- Master’s degree in Social Work (MSW) or Bachelor’s degree with relevant experience.
- Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the State of Texas.
- Minimum of 2-3 years of experience in social services, case management, or a related field.
- Strong knowledge of local community resources, government assistance programs, and healthcare systems.
- Excellent written and verbal communication skills with a high level of empathy.
- Proficiency in Microsoft Office Suite and case management software.
