Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans for clients.
- Provide crisis intervention and emotional support to clients and their families.
- Advocate for clients' rights and access to community resources, healthcare, and housing.
- Monitor client progress and adjust treatment plans as necessary.
- Maintain accurate and confidential case documentation in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams, including medical staff and legal advocates.
Qualifications
- Master’s degree in Social Work (MSW) required; Licensure (LCSW, LMSW, or LSW) preferred.
- Minimum of 2-3 years of experience in case management or social work.
- Strong understanding of local social service systems and resource networks.
- Excellent written and verbal communication skills.
- Ability to handle sensitive information with the highest level of confidentiality.
- Valid driver’s license and willingness to travel to client locations.
