Responsibilities
- Conduct comprehensive biopsychosocial assessments to identify client needs and risk factors.
- Develop, implement, and monitor individualized case plans tailored to client goals and capabilities.
- Coordinate with healthcare providers, housing authorities, and community organizations to secure necessary resources.
- Provide crisis intervention and emotional support during times of acute stress or transition.
- Maintain accurate and confidential case files in compliance with HIPAA and agency standards.
- Facilitate support groups and educational workshops to promote community resilience.
Qualifications
- Master’s degree in Social Work (MSW) or related field (Bachelor’s with significant experience considered).
- Current State Licensure (LCSW, LMSW, or equivalent) required.
- Minimum of 3 years of experience in case management or social services.
- Strong knowledge of local social service systems, healthcare regulations, and community resources.
- Excellent verbal and written communication skills, with a focus on empathy and active listening.
- Demonstrated ability to work effectively in a high-paced, multidisciplinary team environment.
