Responsibilities
- Conduct thorough psychosocial assessments and develop individualized care plans tailored to client needs and goals.
- Provide direct clinical counseling, crisis intervention, and emotional support during times of acute distress.
- Advocate for clients within healthcare, legal, and social service systems to ensure access to essential resources.
- Collaborate with interdisciplinary teams, including medical professionals and legal aid, to coordinate holistic care.
- Maintain accurate, confidential, and timely documentation in compliance with HIPAA and agency standards.

Qualifications
- Master’s degree in Social Work (MSW) required; valid New York State LCSW or LMSW license.
- Minimum of 3-5 years of experience in clinical social work or community-based case management.
- Strong interpersonal and communication skills with a demonstrated capacity for active listening and empathy.
- Ability to handle sensitive information with the highest level of confidentiality and integrity.
- Familiarity with New York State social services regulations and community resources.
- Proficiency in electronic health records (EHR) and case management software.
