Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans.
- Provide clinical case management and crisis intervention support to clients and families.
- Advocate for client rights and connect individuals with community resources and government programs.
- Maintain accurate and confidential case documentation in accordance with regulatory standards.
- Collaborate with interdisciplinary teams, healthcare providers, and legal professionals.
Qualifications
- Masters of Social Work (MSW) degree from an accredited university.
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum of 3-5 years of experience in family services or community mental health.
- Strong understanding of trauma-informed care and family systems theory.
- Excellent verbal and written communication skills.
