Responsibilities
- Provide crisis intervention, counseling, and case management services to individuals and families
- Develop and implement community outreach programs addressing mental health, housing instability, and food insecurity
- Collaborate with healthcare providers, schools, and government agencies to create integrated support networks
- Conduct needs assessments and create personalized care plans using evidence-based practices
- Advocate for systemic change through policy recommendations and community partnerships
- Maintain accurate documentation and comply with all privacy regulations
- Mentor junior staff and facilitate trauma-informed training workshops
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 3 years of direct client experience in community-based social services
- Registered with the Ontario College of Social Workers and Social Service Workers
- Certification in Trauma-Informed Care and Crisis Intervention preferred
- Experience working with diverse populations including LGBTQ+, refugees, and individuals experiencing homelessness
- Strong knowledge of local social service systems and available resources
- Excellent communication skills with ability to navigate complex family dynamics
- Valid Ontario driver's license and reliable transportation for community visits
