Responsibilities
- Provide crisis intervention and long-term case management for at-risk populations including refugees, survivors of domestic violence, and individuals experiencing homelessness
- Develop and implement personalized care plans using evidence-based assessment tools and trauma-informed practices
- Collaborate with interdisciplinary teams including healthcare providers, legal aid, and housing specialists to create holistic support networks
- Lead community outreach programs and workshops focused on mental health literacy, financial literacy, and life skills development
- Advocate for policy changes and systemic improvements through research, report writing, and stakeholder engagement
- Maintain accurate documentation while adhering to CASW standards and provincial privacy regulations
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 5 years of progressive experience in direct social services with vulnerable populations
- Registered member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) or equivalent
- Demonstrated success in case management, crisis intervention, and community advocacy
- Proficiency in electronic health records and data analysis tools
- Fluency in English with proficiency in at least one additional language highly valued
