Responsibilities
- Develop and implement comprehensive case management plans for diverse client populations
- Provide crisis intervention, trauma-informed counseling, and advocacy for vulnerable adults and families
- Collaborate with multidisciplinary teams including healthcare providers, legal advocates, and housing specialists
- Conduct community outreach to identify at-risk populations and connect them with essential services
- Maintain meticulous documentation ensuring compliance with HIPAA and agency protocols
- Lead program initiatives addressing systemic barriers to social equity and economic stability
- Mentor junior staff and supervise social work interns in field placements
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited institution
- NYS LMSW/LCSW licensure required
- Minimum 5 years progressive experience in direct practice with vulnerable populations
- Expertise in crisis intervention, motivational interviewing, and trauma-informed care
- Proven success in navigating complex community systems (housing, healthcare, legal)
- Advanced knowledge of federal/state regulations governing social services
- Bilingual proficiency in Spanish strongly preferred
- Experience supervising staff or interns
