Responsibilities
- Develop and implement comprehensive case management plans for at-risk populations including youth, families, and vulnerable adults
- Conduct psychosocial assessments and provide crisis intervention services
- Collaborate with community partners to coordinate resources and support networks
- Advocate for policy changes and social justice initiatives at local and state levels
- Provide clinical supervision to junior social work staff and interns
- Maintain accurate client documentation and ensure compliance with HIPAA regulations
- Lead trauma-informed care workshops for community stakeholders
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW or LMSW license in New York State
- Minimum 5 years of direct clinical experience in social services
- Expertise in crisis intervention and trauma-informed care models
- Strong advocacy skills with experience in policy development
- Proficiency in electronic health records (EHR) systems
- Bilingual proficiency in Spanish preferred
- Valid NYS driver's license with clean record
