Responsibilities
- Provide direct counseling and case management to individuals/families facing homelessness, domestic violence, and mental health challenges
- Develop and implement comprehensive intervention plans aligned with evidence-based practices
- Collaborate with healthcare providers, legal advocates, and community organizations
- Conduct crisis intervention and risk assessments for high-need populations
- Lead community outreach programs and educational workshops
- Maintain accurate client documentation and comply with HIPAA regulations
- Mentor junior staff and contribute to program development initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 5 years of direct service experience in social services
- LCSW or LMSW license in New York State
- Crisis intervention certification (CIT preferred)
- Proficiency in electronic health records (EHR) systems
- Experience working with diverse populations including LGBTQ+ and immigrant communities
- Strong advocacy skills and knowledge of NYC social service resources
- Valid NYS driver's license (required for home visits)
