Responsibilities
- Client Assessment & Advocacy: Conduct comprehensive intakes and psychosocial assessments to identify client needs and develop robust, individualized case plans.
- Resource Linkage: Expertly navigate and connect clients with critical services, including housing assistance, mental health support, employment programs, and financial literacy workshops.
- Crisis Intervention: Provide immediate support during crises, ensuring client safety and de-escalation of volatile situations.
- Documentation: Maintain accurate, timely, and confidential electronic health records (EHR) and case notes in compliance with HIPAA and agency standards.
- Collaboration: Act as a liaison between clients, families, healthcare providers, and government agencies to facilitate seamless care transitions.
- Program Development: Contribute to the continuous improvement of case management protocols and support training initiatives for junior staff.
Qualifications
- Education: Master of Social Work (MSW) preferred; Bachelor’s degree in Social Work (BSW) with 3+ years of experience accepted.
- Licensure: Current New York State LSW or LCSW license (or eligible for licensure).
- Experience: Minimum of 3-5 years of experience in social work, case management, or a related human services field.
- Knowledge: Deep understanding of NYC social service landscape, entitlement programs, and community resources.
- Skills: Strong case management, conflict resolution, and interpersonal communication skills.
- Attributes: Empathy, cultural competence, and the ability to work effectively in a fast-paced, high-demand environment.
