Job Overview:
The Senior Nurse Practitioner will perform comprehensive patient evaluations, formulate treatment plans, and provide ongoing management for patients with various cardiovascular conditions. You will act as a vital liaison between patients and the healthcare team, ensuring seamless coordination of care.
Responsibilities
- Perform comprehensive patient history, physical examinations, and diagnostic testing.
- Formulate and implement individualized treatment plans in collaboration with the medical director.
- Prescribe medications and manage therapeutic regimens within the scope of practice.
- Conduct follow-up visits to monitor patient progress and adjust care plans as necessary.
- Provide patient education regarding cardiovascular health, lifestyle modifications, and medication adherence.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Participate in quality improvement initiatives and clinical research studies.
- Collaborate with the nursing staff and administrative team to ensure efficient clinic operations.
Qualifications
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution.
- Board Certification as a Nurse Practitioner (FNP or Cardiovascular Nurse Practitioner).
- Valid New York State RN and NP licenses.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 3-5 years of clinical experience in a cardiology or internal medicine setting.
- Strong proficiency in Electronic Health Records (EHR) systems (e.g., Epic, Cerner).
- Excellent communication, interpersonal, and patient assessment skills.
- Ability to work independently while contributing to a multidisciplinary team.
