Responsibilities
- Review and abstract medical records from various sources, including electronic health records (EHR) and paper charts, to identify diagnoses and procedures.
- Assign appropriate ICD-10-CM and CPT-4 codes to medical records ensuring 100% compliance with HIPAA and CMS guidelines.
- Perform complex coding scenarios, including inpatient, outpatient, and emergency department records.
- Liaise with physicians and clinical staff to clarify documentation and resolve coding queries.
- Participate in monthly coding audits and quality improvement initiatives to maintain high standards.
- Stay current with changes in medical coding guidelines, payer policies, and regulatory updates.
- Utilize coding software (e.g., 3M Encoder, Epic) to ensure data accuracy and efficiency.
Qualifications
- Minimum of 3-5 years of experience in professional medical coding (Inpatient and/or Outpatient preferred).
- Current and active certification from a recognized body (CPC, CCS, COC, CRC, or RHIA/RHIT).
- Thorough knowledge of ICD-10-CM, CPT-4, HCPCS Level II, and ICD-10-PCS coding principles.
- Strong understanding of medical terminology, anatomy, physiology, and disease processes.
- Excellent analytical skills with a proven track record of accurate coding and documentation.
- Proficiency in using EHR systems and coding software.
- Ability to work independently in a remote setting with minimal supervision.
