Responsibilities
- Conduct comprehensive biopsychosocial assessments and develop individualized, client-centered care plans.
- Facilitate access to essential services including housing, healthcare, and vocational training programs.
- Advocate for clientsâ rights and ensure they receive equitable treatment within the healthcare and social systems.
- Monitor client progress, adjusting support strategies as necessary to ensure positive, measurable outcomes.
- Maintain accurate and confidential documentation in strict compliance with HIPAA and agency standards.
- Collaborate with interdisciplinary teams, including medical providers and legal advocates.
Qualifications
- Masterâs degree in Social Work (MSW) preferred; Bachelorâs degree with significant relevant experience may be considered.
- Current New York State Social Work license (LCSW or LMSW) is required.
- Minimum of 3 years of progressive experience in case management, social work, or community outreach.
- Strong knowledge of local community resources and service providers in the New York metro area.
- Excellent verbal and written communication skills, with the ability to navigate sensitive situations with empathy and professionalism.
- Proficiency in Microsoft Office Suite and case management software.
- Bilingual (English/Spanish) skills are a strong plus.
