Key Responsibilities:
Our ideal candidate will be a proactive problem solver with a passion for community health. Your daily duties will include:
- Conduct Comprehensive Assessments: Perform thorough biopsychosocial evaluations to identify client needs, strengths, and barriers to success.
- Develop Individualized Care Plans: Create and monitor tailored case management plans that align with client goals and community resources.
- Crisis Intervention & Support: Provide immediate support during mental health crises and connect clients with emergency services when necessary.
- Resource Navigation: Act as a liaison between clients and external agencies, helping secure housing, employment, and healthcare services.
- Advocacy: Champion the rights of our clients within healthcare and legal systems to ensure equitable access to services.
- Documentation & Reporting: Maintain accurate, confidential, and timely electronic health records (EHR) and prepare required state reports.
Qualifications:
We are looking for candidates who bring a blend of clinical expertise and genuine empathy. Requirements include:
- Education: Master’s degree in Social Work (MSW) required; LCSW or LICSW license preferred.
- Experience: Minimum of 3-5 years of experience in case management, mental health, or social work.
- Skills: Strong knowledge of mental health disorders, substance abuse treatment, and trauma-informed care.
- Communication: Exceptional verbal and written communication skills, with the ability to build rapport with diverse populations.
- Technical Proficiency: Comfortable using Electronic Health Records (EHR) systems and case management software.
- Availability: Flexibility to work occasional evenings or weekends as required by client needs.
