Responsibilities
- Conduct comprehensive initial assessments and develop individualized, client-centered care plans.
- Provide direct clinical counseling and crisis intervention services to individuals and families.
- Advocate for clients to ensure access to necessary resources, including housing, healthcare, and financial assistance.
- Collaborate with interdisciplinary teams, including medical professionals and legal aid, to coordinate holistic care.
- Maintain accurate, confidential, and timely documentation in accordance with HIPAA regulations and agency policies.
- Facilitate support groups and educational workshops to promote community resilience.
- Monitor client progress and adjust treatment plans as needed to ensure positive outcomes.
Qualifications
- Current New York State Licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) with supervision eligibility.
- Bachelor’s or Master’s degree in Social Work (MSW) from an accredited institution.
- Minimum of 2 years of experience in case management, clinical social work, or a related field.
- Strong understanding of mental health systems, substance abuse resources, and community support services.
- Excellent verbal and written communication skills with the ability to build rapport quickly.
- Demonstrated ability to handle sensitive information with the utmost confidentiality and ethical integrity.
- Proficiency in Microsoft Office Suite and case management software.
