Responsibilities
- Client Assessment & Care Planning: Conduct comprehensive biopsychosocial assessments to identify client needs and develop individualized, evidence-based care plans.
- Clinical Intervention: Provide direct clinical services, including individual and group counseling, utilizing modalities such as Cognitive Behavioral Therapy (CBT) and Motivational Interviewing.
- Resource Advocacy: Act as a liaison between clients and external agencies to secure housing, healthcare, employment, and financial assistance.
- Crisis Stabilization: Manage acute crisis situations, ensuring immediate safety and connecting clients to emergency resources when necessary.
- Documentation & Compliance: Maintain accurate, timely, and confidential clinical documentation in compliance with HIPAA and agency protocols.
- Collaboration: Work alongside interdisciplinary teams, including nurses, physicians, and legal advocates, to ensure a holistic approach to patient care.
Qualifications
- Education: Master’s degree in Social Work (MSW) from an accredited university.
- Licensure: Valid New York State Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) credential.
- Experience: Minimum of 2-3 years of experience in clinical social work or case management within a healthcare or community-based setting.
- Communication Skills: Exceptional verbal and written communication abilities with a proven track record of building rapport with diverse populations.
- Empathy & Ethics: Strong emotional intelligence, cultural competence, and a deep understanding of ethical standards in social work.
- Technical Proficiency: Comfortable using electronic health records (EHR) systems and Microsoft Office Suite.
