Responsibilities
- Conduct thorough psychosocial assessments to evaluate client needs, strengths, and risk factors.
- Develop and implement individualized treatment plans utilizing evidence-based therapeutic modalities.
- Provide direct clinical services, including individual, couples, and family therapy sessions.
- Coordinate and advocate for clients to access necessary medical, educational, and social resources.
- Facilitate psycho-educational workshops and support groups to promote community resilience.
- Maintain accurate, confidential documentation in compliance with HIPAA and agency standards.
- Collaborate effectively with multidisciplinary teams to ensure comprehensive client care.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited university.
- Current New York State Licensure as a Licensed Clinical Social Worker (LCSW) or LCSW Candidate.
- Minimum of 2-3 years of experience in clinical social work, mental health, or case management.
- Strong understanding of community resources and social service systems.
- Exceptional interpersonal, communication, and active listening skills.
- Proficiency in Electronic Health Records (EHR) systems.
