Responsibilities
- Conduct comprehensive psychosocial assessments to identify client needs and develop individualized care plans.
- Facilitate case management services, including crisis intervention and safety planning.
- Advocate for clients within healthcare, legal, and social service systems to secure necessary resources.
- Maintain accurate, confidential, and timely documentation in accordance with HIPAA standards and agency policies.
- Collaborate with interdisciplinary teams to coordinate holistic treatment approaches.
- Provide psychoeducation and support to clients and their families regarding mental health and community resources.
- Monitor client progress and adjust care plans as necessary to ensure positive outcomes.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related field required.
- Valid LCSW, LMSW, or LPC license in New York State is preferred.
- Minimum of 3 years of experience in clinical social work or case management within a healthcare setting.
- Strong knowledge of community resources, mental health diagnoses, and treatment modalities.
- Excellent verbal and written communication skills with the ability to convey complex information clearly.
- Proficiency in electronic health records (EHR) and Microsoft Office Suite.
- Demonstrated ability to work effectively in a fast-paced, high-stress environment.
