As a Case Manager, you will be the bridge between clients and the services they need. We are looking for a professional who is not only skilled in case management but also deeply committed to social justice and community health.
Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized, strengths-based service plans.
- Facilitate access to essential resources, including housing, healthcare, and employment services.
- Monitor client progress and adjust care plans in response to changing needs or crises.
- Advocate for client rights and ensure equitable access to community resources and programs.
- Collaborate with interdisciplinary teams, healthcare providers, and external stakeholders to coordinate holistic care.
- Maintain accurate, confidential, and timely documentation in accordance with HIPAA and agency standards.
- Facilitate group therapy sessions or support groups to foster community resilience.
Qualifications
- Bachelor’s degree in Social Work (BSW) or a related field (Master’s degree in Social Work (MSW) preferred).
- Current LMSW or LCSW licensure in New York State.
- Minimum of 2-3 years of experience in case management or direct social services.
- Strong knowledge of local social service systems, community resources, and funding streams.
- Proven ability to handle sensitive information with strict confidentiality and discretion.
- Excellent verbal and written communication skills with a focus on clarity and empathy.
- Proficiency in Microsoft Office Suite and case management software (e.g., Salesforce, eClinicalWorks).
