Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans tailored to client needs.
- Provide direct counseling, crisis intervention, and emotional support to clients facing housing, financial, or mental health challenges.
- Advocate for clients to ensure access to essential resources, including healthcare, legal aid, and employment services.
- Collaborate with multidisciplinary teams, including medical professionals and legal counsel, to ensure coordinated care.
- Maintain accurate and timely documentation in electronic health records (EHR) in compliance with agency and regulatory standards.
- Facilitate support groups and educational workshops to foster community building and skill development.
- Monitor client progress and adjust treatment strategies as necessary to ensure positive outcomes.
Qualifications
- Master’s degree in Social Work (MSW) required; Licensure (LMSW or LCSW) preferred.
- Minimum of 2 years of experience in case management, social work, or a related human services field.
- Strong knowledge of community resources, social service systems, and legal entitlements.
- Excellent verbal and written communication skills with a focus on active listening.
- Cultural competency and the ability to work effectively with diverse populations.
- Ability to handle sensitive information with strict confidentiality.
- Proficiency in Microsoft Office Suite and case management software.
