Responsibilities
- Develop and implement comprehensive care plans for clients facing housing instability, mental health crises, and poverty
- Collaborate with multidisciplinary teams including therapists, healthcare providers, and legal advocates
- Conduct psychosocial assessments and crisis interventions in high-stakes community settings
- Oversee caseload of 25+ clients while maintaining documentation compliance with NY state regulations
- Design and lead trauma-informed support groups for diverse populations
- Advocate for policy changes through community organizing and public testimony
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LMSW or LCSW license with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care
- Experience managing complex cases involving homelessness, domestic violence, or developmental disabilities
- Strong data analysis skills for outcome measurement and program evaluation
- Bilingual proficiency in Spanish preferred
- Valid NY driver's license with clean record for home visits
