Responsibilities
- Provide immediate crisis intervention and emotional support to clients in the community.
- Conduct comprehensive initial assessments and create personalized care plans.
- Monitor client progress and adjust support strategies as needed.
- Liaise effectively with healthcare professionals, social services, and family members.
- Document all interactions and case notes accurately and confidentially.
- Advocate for client rights and ensure access to necessary resources.
Qualifications
- Relevant Bachelor’s degree in Social Work, Psychology, or a related field.
- Valid UK Driving License and access to a vehicle.
- Experience working with individuals with mental health conditions or learning disabilities.
- Understanding of safeguarding policies and legislation.
- Strong interpersonal skills with the ability to build trust quickly.
- Proficiency in Microsoft Office and case management software.
